About Whole Family Health at Belmar
This notice is to inform you that Whole Family Health at Belmar will close as of Aug. 31, 2016. We have valued the services that these providers have offered and are confident in the high-level of care that you’ve received to date. Dr. Brieanna Seefeldt will continue to see patients until July 29 and Drs. Lisa Nguyen and Erica Liesmaki will see patients until Aug. 17.
As we move forward, we want to ensure that you are taken care of and given the highest quality of care. We are happy to share with you these physicians’ new practice locations once that is made available. In the meantime, below is a list of trusted primary care and OBGYN providers that we recommend for you to learn more about to see if they may be the right fit for you. As you explore your healthcare options, please reach out if you have any questions or concerns.

Your medical record will stay with HealthONE. Once you choose a healthcare provider, your record will be made available to you by completing the Medical Records Release form and faxing it to (303) 238-5832. Or, you may call (303) 268-1577 until Feb. 28, 2017. Following that date, please call (303) 584-8000 for further instructions on how to obtain your medical record. Alternatively, for information regarding other primary care or OBGYN physicians in your community, please contact your health insurance carrier.
If you have a future appointment scheduled (after Aug. 17, 2016) at Whole Family Health, you will be contacted by our office to cancel the appointment. At that time, if you have selected another healthcare provider, we will obtain the appropriate information to begin a transfer of your medical records copy to your new healthcare provider. We truly apologize for any inconvenience this change may cause, but are committed to providing the highest quality of care and will do our best to accommodate your needs.
